Make sure your Beneficiary Information is up to date!

In order for Professional Musicians, Local 47 and Employers’ Health and Welfare Fund (the “Fund”) to best serve your needs, we need to have your current and accurate personal information on file. And while you may not think about it all the time, whom you designate as your beneficiary is crucial information.

If an eligible member doesn’t complete the beneficiary form, he or she will not be entitled to the Life and AD&D (Accidental Death or Dismemberment) Benefit, which can be as much as $20,000. This is why a beneficiary must be designated.

It is also important for eligible members to update their beneficiary form when they have a life event such as marriage/divorce or birth of a child, because whoever is on the beneficiary form is who the carrier will pay the proceeds of the benefit.

Beneficiary forms are meant to be a straightforward tool, but sometimes people forget what they’ve written on the beneficiary forms and fail to update them when they should have.

You should fill out a new form if you’ve had a birth, death, marriage or divorce in your family. Mail your completed, signed beneficiary form to the Fund office at:

Be sure to keep a hard copy for your files. Also, make sure important information, like your address and Social Security number, your covered dependents’ Social Security numbers and all phone numbers, is up to date. Review any communications you receive from PacFed to be sure everything is correct.

You can change your beneficiary at any time, but to do so you must fill out a new form and submit it.

Even if you have notified your union and employer of changes, you still must inform the Fund. This is not done automatically.
