Make sure PacFed is up to date with your personal information
For PacFed’s staff to best serve your needs, your current and accurate personal information must be on file. Even if you have notified your union and employer of changes, you still must inform PacFed. None of this is done automatically, so please be proactive with your benefits.
Be sure that important information, including your address, phone number, email address and Social Security number, as well as your covered dependents’ Social Security numbers, are up to date. Review any communications you receive from PacFed to ensure everything is correct.
This is true at all times, whether it’s during your company’s Open Enrollment Period or when you’re undergoing a change in life circumstances, such as a births, deaths, adoptions, marriage or divorce. For example, if you get married, your new spouse may be eligible to share in your benefits. As soon as you say “I do,” obtain a Change Form from your employer’s HR department or the Trust Fund Administration Office, and return the form with a copy of your marriage certificate within 30 days of the marriage date.
If your family is expecting a baby, adopting a child or having a child placed for adoption, send a completed Change Form, along with a copy of the birth certificate or adoption papers, to the Trust Fund Administration Office to begin coverage. You must contact PacFed within 30 days of the birth of a new baby to ensure that the baby is covered. Your new baby is automatically covered by your medical benefits for the first 30 days only.
If you have a domestic partnership, certain documents are required to add a partner to your plan. Contact PacFed to determine which forms are required for your domestic partnership.
Update your beneficiary!
While you may not think about it all the time, whom you designate as your beneficiary is crucial information. Beneficiary forms are meant to be a straightforward tool for heirs to bypass the probate process and receive their funds in a timely manner. But sometimes people forget what they’ve written on the enrollment forms and fail to update them when they should have.
You should fill out a new form if you’ve had a birth, death, marriage or divorce in your family. If you can't find your beneficiary designation form, ask for a new one. You can change your beneficiary at any time, but to do so you must fill out a new enrollment form and submit it.
